Community Health Outreach Program 

The Town Of Parker's Community Health Outreach Program formally known as PHEPP, is focused on improving the health of adults and youth by offering education and support to the communities of La Paz through the creation and promotion of healthy community environments and our menu of health and wellness activities. 
Health Education Programs:
Adolescent Health Classes (Youth 11-19)
Active Parenting of Teens
Teen Solutions: STAND Youth Coalition (Youth ages 12-19)
Tai Chi for Fall Prevention (Seniors)
Finding Meaning & Hope: Alzheimer's and Dementia Group (Family Caregivers)
Assistance with Tobacco Dependence
Food for Thought: Healthy Cooking Demos
Parker Community Garden
Arizona Retail Tobacco Training (Local Retail Stores)
Child Safety Seat Course (Families) 
​Join a group of fellow family caregivers to discuss how to regain hope and build resilience when dealing with the complexities of ongoing loss associated with caring for someone with Alzheimer's and other dementias. In Duet's Finding Meaning and Hope workshop, you will learn skills to help you stay strong, healthy, resilient, and positive as you navigate your caregiving journey with healing and hope.
This is a 10-week FREE workshop that will be offered virtually. To participate, you will need a computer, tablet, or smart phone with a camera and sound capabilities. Contact us for more information.
